You don’t need a doctor’s letter (I’ve checked this with local surgery). You can self certify that you feel ‘extreme distress’ wearing a mask, in the very unlikely event of being quizzed by the police. Otherwise, no requirement to explain yourself to shop staff or the public.

The notice can be hung on cord round neck and with its NHS colour taps into the hallowed deference many seem to have to the medical monolith that passes for healthcare in the UK.
